Registration

All participants are requested to register online in advance. All speakers are required to register by May 31, 2016 in order to make a presentation. The Pre-Confirmation e-mail with the registration details will be sent to you after your online registration is complete and a Confirmation e-mail will be sent to you after the payment has been confirmed. Please bring the print-out of the Confirmation e-mail with you to the Congress.

Inclusions

*1 These registration fees include: Session Participation, Congress Kit including Congress Bag and Program.

*2 Delegate are members of National Member Associations.

*3 Students must attach a proof of full-time student status such as a letter from you Academic Center or copy of student photo ID. Students in Japan are requested to attach their photo ID of the designated schools.

*4 Accompanying persons should be the spouse of the registrant.

To qualify for the Early Bird Registration Fee, the application and payment must be completed by no later than May 31, 2015.

Cancellation Policy

No refunds will be made for cancellation of your registration including GALA dinners and other optional programs.

Passport and Visa

A valid passport is required to enter Japan. Participants from certain countries may be required to obtain a visa to enter Japan.
If you are uncertain about your requirements, please consult your nearest Japanese Embassy or Consulate, or visit the website of the Ministry of Foreign Affairs of Japan (http://www.mofa.go.jp/j_info/visit/visa/index.html).
The Registration Office will send an official letter of invitation upon request, through the registration form below after the registration and payment is complete.
If you require a visa to enter Japan, please register first. The Registration Office will then make an invitation letter and a guarantee letter based on the information that you provide in the Visa Information Sheet and send them to you at the address specified on the sheet.
Please make your own application for visa acquisition at the Japanese Embassy or Consulate once you receive the letter.

* If you need to obtain a visa, please apply 3 months in advance of your visit.
IFHIMA2016 Organizers, Secretariat, and the Registration Office are not responsible for visa issuance.

Accommodation

JTB Global Marketing & Travel Inc. has been appointed as an official travel agency for the congress and will handle hotel accommodations. JTB Global Marketing & Travel Inc. has reserved a sufficient number of rooms at the following hotels for the participants during the congress period.
Hotel assignment will be made on a first-come, first-served basis.
